Evergreen is a daytime program in Stamford for older adults — including those experiencing cognitive change — who may benefit from structure, activity, social connection and support during the day while continuing to live at home.

We work closely with neurologists, geriatric care managers, home care agencies and hospital social workers, and we are honest with families when Evergreen is — and isn’t — the right fit.

Who Evergreen may be a fit for.

Evergreen may be worth exploring for families navigating situations such as:

  • Mild cognitive impairment
  • Memory loss or other cognitive change
  • Increasing isolation at home
  • The loss of a daytime routine
  • A need for meaningful social engagement
  • Family caregiver strain
  • A family seeking daytime support while their loved one continues living at home

This is not a clinical eligibility list, and it isn’t exhaustive. Evergreen doesn’t diagnose or provide medical care. If you’re unsure whether a particular family would benefit, call us and we’ll talk it through.

Introduce us.

You or the family contacts Evergreen — a call, an email, or the form on this page.

We talk with the family.

We learn about the person, their needs, their day today, and what the family is trying to solve.

Experience Evergreen.

When it seems appropriate, we invite the prospective member to spend a Complimentary Day with us.

Determine fit together.

If Evergreen feels right for the member and the family, we help them settle on a schedule and next steps. If it isn't the right fit, we'll say so.

A referral isn’t an admission. We assess fit together with the family, and we won’t enroll someone whose needs we can’t meet well.
